OVERVIEW OF COVERAGE

All firefighters are covered against workplace accidents, including a training exercise, a maneuver or testing firefighting equipement.

 

COMPULSORY BENEFITS

Accidental death
and dismemberment (AD&D)

 

Loss of sight, speech or hearing

 

Paralysis

 

Loss of use of a limb

 

 

 

 

 

If the loss occurs within 365 days following the date of the accident, the coverage provides a benefit payment up to:
• $250,000 in the case of accidental death;
• $500,000 in the case of paralysis;
• $250,000 for any other loss.
Benefits are paid according to a predetermined benefits table.

  

OPTIONAL BENEFITS 

Cardiac or circulatory dysfunction

If the insured person dies, he or she will receive all or a part of the following benefits:
• a death benefit of up to $250,000;
• a weekly benefit paid for up to 104 weeks;
• reimbursement of medical expenses.

Total and permanent disability

Benefit can go up to $250,000 if the injury results in the insured person’s total and permanent disability.

Weekly indemnity in case of accident

For a total disability, the weekly benefit can be up to $500 for one, two or five years according to the option chosen. For a partial disability, the weekly benefit can be up to $250 for eight weeks. Benefits from this program plus any other benefits paid from any other source cannot exceed 80% of pre-disability gross salary.

 

Weekly indemnity for homemakers

The weekly indemnity can go up to $100 for a maximum of 13 weeks. This indemnity will be paid if the insured person is injured and unable to carry out the tasks related to homeworking or education of children for 7 consecutive days.

Reimbursement of medical expenses
following an accident

 

The following medical fees are reimbursed up to $10,000:
• prescription drugs;
• ambulance service;
• semi-private hospital room;
• physiotherapist or chiropractor care;
• private nursing care;
• purchase of crutches, braces, orthopedic devices and trusses.

Dental care following an accident

Benefit can go up to $1,000 in the case of an accident.

Dental prostheses or bridges

In case of an accident, replacement or repair of removable dentures fixed bridges and/or crowns up to $500.

 

 

ADDITIONAL CUSTOMIZED BENEFITS 

Benefit in case of fracture          

In case of a fracture or dislocation of one or several bones due to an accident, the benefit can be up to $1,000.

Disfigurement    

In case of a disfigurement due to a third degree, the benefit can be up to $250,000.

Infectious illness

If the insured person dies following accidental exposure to an infectious illness, the coverage includes payment of the following benefits:
· a death benefit of up to $250,000 ;
· a weekly benefit paid for up to 104 weeks ;
· reimbursement of medical expenses.

Eyeglasses or contact lense

Pays up to $200 for purchase of eyeglasses or contact lenses as a result of an injury when neither previously required or worn.

Transportation after death

The benefit can be up to $10,000 for return of the body.

Funeral expenses

Following the accidental death of an insured person, the benefit can be up to $4,000 and includes the cost of services and/or supplies necessary for the burial of the body.

Study benefits

In the case of accidental death of the insured person, the payment of a study benefit of up to $5,000 per year per child who is attending university, Cegep or a professional and technical training school for up to 4 years.

Childcare expenses

In the case of accidental death of the insured person, the payment of childcare expenses of up to $5,000 per year per dependent child who goes to a daycare for up to 4 years.

Professional rehabilitation

Benefit for professional rehabilitation can be up to $10,000 if the injury occurs within 3 years following application for this benefit.

Professional training

In case of the insured's death, the payment of a benefit for professional training can be up to $10,000.

Transportation for family reasons and
accommodation

If following an injury, the insured person is hospitalized more than 150 km from his or her residence for at least four days, the benefit for travel and accommodation expenses for one
or several family members can be up to $1,000.

Adjustment of the residence and/or vehicle

If following an accident, the insured person is mutilated in both legs or both feet or loses use of these, the benefit for home, vehicle and/or wheelchair adaptation can be up to $10,000.

Hospital benefit

Any injury requiring hospitalization by the insured person for at least four days entitles the participant to a hospital benefit of up to $2,500 per month payable starting on the fifth day.
(If the injury is covered under the "Benefits in case of accidental loss" provision, the payment is made on the first day of the hospitalization.)
